After a visit, the next close friend needs an exploitable note: what has been done, the possible action that follows and the person who accepted it. This model focuses on the after-visit. To prepare in advance the tasks of a next of kin who arrives, use the form instead. prepare a relay before the next visit.
A short format to transmit the essentials
You can copy this frame to a notebook, a shared note or a coordination tool:
Tram blank to copy
Date and time: ...
Author: ...
Useful facts, in concrete terms: ...
Action to be taken (or none): ...
Responsible person who accepted: ...
Next relay and date: ...
| Field | To be provided |
|---|---|
| Date and time | When the visit or passage took place |
| Author | Name of person who writes |
| Useful facts | What has been done or seen, in simple terms |
| Action to be taken | The next practical step, if it exists |
| Responsible | Person who confirms taking the task |
| Next relay | Who will be informed or then pass, and when |
Keep a clear separation between fact and interpretation. "The list of races has been updated" is factual. "Everything is fine" is a general conclusion that may be unclear. If information is missing, write "to be confirmed" and indicate who will verify it.
| Wave formulation | Note more exploitable |
|---|---|
| "It's all right" | "The list of races was updated after the visit on 14 May." |
| "We should call" | "Call to pass Friday; Marc confirmed that he is in charge." |
| "The schedule is Tuesday" | "The calendar indicates Tuesday; the hour remains to be confirmed by Marc." |
Do not transform the reformulation into an interpretation: keep only what has been observed, achieved or agreed upon. If no relative has accepted the continuation, note "responsible to confirm" rather than assigning the task implicitly.
Fictitious example to be adapted
Date: 14 May 2026, 17.30
Author: Sophie, girl
Facts: Sophie brought the races requested by her father. The paper calendar indicates an appointment next Tuesday, but the schedule is not noted.
Action to be taken: check the schedule from the document received by the family.
Responsible: Marc, who confirmed that he would do the check on Friday.
Next relay: Marc calls his father on Saturday to confirm the time and complete the family calendar if the information is verified.
This example is entirely fictitious. It shows how to assign a single next action and avoid two relatives thinking that the other is taking care of it. It does not contain any clinical details or replace an exchange with a professional when a professional question arises. If Marc had not accepted, the note would say "responsible to confirm" and would specify who should ask him.
Write a clear and respectful note
A useful transmission is readable by the person concerned and the intended recipients. Prefer a short sentence per action, avoid judgments and do not copy private information that is not necessary for the relay. Before sharing, check that your parent agrees with the use of the medium and that the people who have access to it are those who need it.
The public statement on the care journal recommends factual, dated and shared transmissions with the consent of the person.
Who's transmitting the note to?
Set the list of recipients with your parent. A shopping task does not necessarily require all the relatives to receive every detail of the visit. If a home worker is concerned, check with the structure and the person receiving care which channels and information are appropriate. Tribulys presents its information dedicated to the speakers on the page Professionals.
Choose a medium that everyone can find
The model can be printed and left at home, stored in a notebook or entered in a shared tool. The important thing is that relatives know where to look and how to report that a task is completed. Avoid dispersing updates between several discussion groups, agendas and documents that are not synchronized by family members.
If an action changes after the visit, add a dated update instead of erasing the previous information without explanation. For example: "Marc confirmed the schedule on May 15; schedule updated by Sophie". If the action is no longer necessary, write that it is closed and by whom the decision has been confirmed. The next of kin who consults the note must be able to distinguish old information from information that is still in force. For changes of passage, the guide on cancellation and family planning details the specific steps.
To rate the transmissions, see digital care journal and Tribulys offers.
FAQ
Should the transmission be detailed?
No. Note what helps the next relay to act: date, author, useful fact, action and responsibility. A brief note is often easier to keep up to date.
Can we record any unverified information?
If it is useful to the organization, mark it "to be confirmed" and assign the audit to an individual. Do not present it as an established fact.
Should we share every note with the whole family?
Not necessarily. Agree with your parent who needs what information and limit the recipients to what is needed.
