This sheet is used to prepare the next passage before it takes place: confirm the niche, agree on the accepted tasks and know who to ask for a clarification. It does not replace the report to be written after the visit; for this, use the transmission model after passing through.
Prepare the card with the parent
Before filling in a shared medium, explain to your parent what you propose to note, who will be able to view it and how the information will be updated. Ask for his or her consent and respect his or her preferences on the recipients. public statement on the care journal recommends factual, dated and shared transmissions with the consent of the person.
The relay form to be completed
| Heading | Information to note |
|---|---|
| Date and niche of the relay | Day, scheduled time and person passing |
| Planned tasks | Practical actions agreed with the parent |
| Last transmission to consult | Where to find it, date of update |
| To be confirmed | Information expected, person responsible for checking |
| Useful contacts | Contact details that the parent wishes to share for the organization |
| Next relay | Who then passes or calls, and when |
| Change or absence | Person to be notified and expected confirmation |
Stay concrete. "Check that the list of races is ready" is clearer than "see races". Do not add a sensitive item by habit. If a detail does not help the relay, it probably does not need to be entered in the card. Never publish access codes, passwords or sensitive data in this model. For instructions related to a professional intervention, refer to the indications of the service and do not replace them with improvised family notes.
Checklist before leaving
- Are the niche and location confirmed with the parent?
- Has the relative accepted the tasks that are proposed to him?
- Does he know where to find useful practical information?
- Does the person concerned know who is coming and how to warn in case of change?
- Has the next point or relay agreed?
If a point remains uncertain, identify the person who will check it and when. The form should not suggest that a relay is insured as long as the incoming relative has not confirmed.
A fictitious example
Relay planned: Sunday, October 18, 2 p.m.
Close that passes: Amine
Agreed task: bring the products on the list prepared by the parent
Already done: updated list Friday by Leïla
In case of a delay: Amine warns the parent
Next relay: Leïla's call Monday night, if the parent wishes
This example is fictitious. The responsibility is explicit: Amine passes; Leïla calls on Monday; in case of delay, Amine warns the parent. The relay is not based on a assumption that another family member will read a message.

Define who informs and who confirms
For each change in schedule, specify the person who must notify the parent and the person who confirms that the relay is well taken into account. If the replacement did not respond, the task remains to be confirmed. This distinction avoids considering an application sent as an accepted task.
If professionals intervene at home, do not assume that the family file is their communication channel. Ask the parent and the service what can be transmitted, to whom and by what means. Professionals presents the context of its tools for professionals; consult the functions presented to choose a support tailored to your organization.
Keep a single version up to date
The card can be printed or stored in a shared tool. Choose a known location of the relatives concerned and indicate the date of last update. If a change is agreed by phone, then update the card and report it to the people who rely on it.

FAQ
How often do you update the form?
Update it when a task, date or maintainer changes. A visible update date helps to locate an older version.
Does the card have to stay at home?
Choose the right medium with your parent: home paper, shared note or digital tool. Support must be accessible to authorized persons and easy to correct.
What if a relay is not confirmed?
Indicate "to be confirmed", the person responsible for restarting and a response deadline. Do not present the task as covered until a relative has agreed.
