France’s ALD care protocol: understanding and organising follow-up

The ALD care protocol is the responsibility of the doctor and the Sickness Insurance. The relatives can arrange appointments and relays around the prescribed follow-up.

ALD care protocol: understand and organize monitoring

A long-term condition can multiply appointments and documents to follow. For the family, it is useful to distinguish administrative recognition from ALD, medical protocol and daily planning. This distinction avoids confusing a list of family tasks with a care decision.

Who establishes the treatment protocol?

The attending physician establishes the protocol with the person and forwards it to the Sickness Insurance. This document specifies the necessary treatment and follow-up as well as the foreseeable duration of care. The agreement is the responsibility of the medical officer of the Sickness Insurance.

There are exonerating and non-exonerant ALDs. The recognition of an ALD does not mean that any expenses are fully reimbursed: the care depends in particular on the care concerned and the rules in force. For a question about a drug, a transport or a leftover charge, ask the professional and the caisse for confirmation.

Prepare questions for the appointment

With the consent of the relative, bring together questions concerning the organisation: next appointments, exams to be programmed, documents to be given and contact to be reached in case of difficulty. The doctor specifies the medical elements; the carers then take care of the practical actions entrusted to them.

Avoid copying an old directive without checking that it remains applicable. If two documents appear to contradict each other, ask the professional which version to use. A decision reported orally by a family member may require clarification.

Build a follow-up schedule

  • Enter the confirmed appointments and their location.
  • Specify who accompanies the person and who organizes the transport.
  • Note the documents to be prepared and their location.
  • Assign calls to an identified person.
  • After the appointment, update the upcoming actions.

The preparation guide for a medical appointment offers an additional checklist.shared calendar allows to distribute the accompaniments and see the slots still without relays.

Keep the medical document confidential

The protocol contains health information. It must be kept and presented to the professionals concerned according to the information received. A family coordinating area is not intended to replace this document or the medical file.

In the care journal, you can choose the information you need from the relay: an appointment made, a document handed over, a question to be transmitted and the next call. Choose the recipients with the accompanying person. For the organisation of a chronic illness, see also our guide for carers.
